Severe Disability Assessment (SDA)

The Severe Disability Assessment (SDA) provides an assessment of your disability status conducted by a Ministry of Health (MOH)-accredited severe disability assessor. This will confirm if one is eligible to make a claim from the severe disability insurance schemes.

You may also be under Supplements offered by a MOH-appointed private insurer to provide additional coverage on top of the basic ESH or CSHL plan.

The assessment will be performed in-clinic at Keystone Clinic & Surgery (Tanah Merah) and will be charged at $100 per assessment, inclusive of GST (as of June 2025). Take note that the prevailing assessment fees are prescribed in subsidiary legislation to the CareShield Life and Long-Term Care Act (2019).

Assessments are strictly by appointment only, please make an appointment prior to going to the clinic. Do note that we do not offer housecalls at this point in time, and tele-assessments are strictly not allowed.

Who is This For?

This assessment is for individuals who wish to confirm their disability status. Individuals are considered severely disabled if they are assessed to be unable to perform at least three out of six activities of daily living (ADLs):

What to Expect?

A typical SDA will typically last about 45 minutes or longer.

If your care recipient requires a mental capacity assessment to be completed to obtain certification for the purpose of applying for AIC schemes, please explicitly request for it when making the appointment. Do note that mental capacity assessments can only be completed by doctors.

​​Before commencing the assessment, our doctor will ask if you have any past medical history or existing medical conditions. You should bring along any prior medical reports, discharge summaries or doctor’s memo. Do also inform the doctor if the assessment is for a new scheme application, or a disability review. Depending on the situation, you may also need to bring along a Disability review letter and/or completed claim form(s) for Supplements.

After giving consent, doctors will review an individual’s medical history and observe the performance of specific tasks to evaluate their ability to carry out these activities. If required, your caregiver will also be asked to show how he/she assists you as you perform the activities.

AIC will reimburse you the assessment fee ($100) which you have paid if you are assessed to be severely disabled. Do wait for AIC or the private insurers to notify you of the scheme(s) application outcome. Our doctors are not allowed to disclose the results of the disability assessment to you.

For CareShield Life claims, the assessment fee for the first disability assessment will be waived, regardless of whether you are assessed to be severely disabled, or whether the claim is successful.

Periodic reviews are carried out to check on some individuals’ severe disability status to ascertain their continued eligibility for payouts. In such cases, AIC will inform you, and assessment fees for such disability reviews will be waived.
